Helpful Hints
Things to remember for your appointment:
• Arrive 15 minutes prior
to your appointment.
• Bring insurance cards.
• Bring currrent glasses.
• Bring a list of current medications, including vitamins
and herbal supplements.
• Arrange for transportation home (if needed).
• If patient is a minor a parent must be present.
• All applicable co-pays and deductibles are due at
time of service.
Prescription Refills
If you need a prescription refilled please ask your pharmacist to fax the office with the request. Faxes can be sent to the San Luis Obispo office at (805) 781-9013 and the Templeton office at (805) 434-5973. Please do not contact the office yourself. Allow at least 48 hours for a refill to be granted. Please make all prescription requests during appointments.
Fees and Insurance
San Luis Eye Associates accepts most HMOs and Preferred Provider Organization’s including Vision Services Plan (VSP) and Medical Eye Services (MESC). Contact your insurer directly to confirm our participation in your plan and for questions regarding terms of coverage. Patients are responsible for all co-pays, deductibles, and services not covered by their insurance. There is a minimum charge of $25 for copying medical records or filling out forms not directly related to insurance billing or referrals. If you have this type of request, please give all forms and instructions to the receptionist. Fee must be paid prior to release of forms or records, and two weeks should be allowed for completion.
Appointments
For your convenience, all doctors and services are available at both offices. To make an appointment please call the office you would like to visit Monday through Friday between the hours of 8am and 5pm. The San Luis Obispo office can be reached at (805) 781-3937 and the Templeton office at (805) 434-5970. In order to better serve busy patients, Optometrist Joe Horne offers extended hours at both locations. Please call for details.
Cancellation Policy
If you must miss your appointment, please call to cancel as soon as possible. This allows us to attend to other patients sooner. Failure to notify us of a cancellation at least 24 hours in advance may result in a charge.
After-Hour Emergencies
If a problem arises outside normal business hours please call (805) 781-3937 to speak to the doctor on-call.
